Windows admin/user password reset

If you boot a Windows system up using a USB boot disk such as Puppy Linux or Knoppix as root, is it relatively straight forward or even possible to set/reset the admin or a user password of the Windows system on the hard-drive (as an innocent means of recovery, naturally)?

It depends on the version of Windows and the type of account log-in.
It used to be easy.
