Disk I/O issue

Hello,

Need your advise to understand more about the disk IO issue.
When we troubleshoot disk IO issue, how do we confirm whether the issue is occurred due to disk or application workload?

which parameter we need to check from iostat output to confirm the exact reason for the slow-io?

Also how to find which process/file is causing the I/O slowness..
